(modified) of Sarubbi and Hull.
The first dose should be a modified loading dose equal
to the maintenance dose in a subject with normal renal function: Loading dose: 1.5-2.0 mg/kg (gentamicin,
netilmicin, sisomicin, tobramycin).
Loading dose: 5-7.5 mg/kg (amikacin, kanamycin, streptomycin).
The left column represents the creatinine clearance in
mL/min; the three right columns show percent of initial dose at given dosing
interval :

Hemodialysis: 1/2 initial dose as a supplement at the
end of dialysis

The maintenance dose should be increased by
approximately 40% in morbidly obese patients.
